Health & Medical Questionnaire

Important notice for all students : Ahead of your upcoming retreat, all participating students are required to complete a confidential Health & Medical Questionnaire prior to departure.

To ensure the safety, well-being, and proper support of every student throughout the trip, our team must have an accurate, up-to-date health profile on file.

Key Guidelines for Completing the Form

Comprehensive History Required: Please disclose all past surgical operations, hospitalizations, and serious medical conditions — regardless of how long ago they occurred.

Current Conditions & Allergies: Include any ongoing medical conditions, dietary restrictions, severe allergies (including anaphylaxis triggers), and current daily medications.

Confidentiality: All medical information provided is kept strictly confidential and accessed only by designated medical and retreat leadership staff for safety purposes.

Submission Deadline: The completed questionnaire must be submitted no later than a month before the retreat. Students without a completed form will not be permitted to attend the retreat.

IMPORTANT: All students must make sure they have any prescribed medicine with them and to include at least an extra week's worth of medicine as sometimes travel disruptions/ delays can happen.
